MISHAWAKA - Sister M. Jacqueline Kozlowski, OSF, 62, of 1515 Dragoon Trail, died Sunday in Saint Joseph Regional Medical Center, South Bend.

Survivors include a sister, M. Ann Christine of Mishawaka; and two brothers, Ronald Kozlowski of Columbus, Ohio, and Raymond Kozlowski of Hammond.

Services will be at 10:30 a.m. Wednesday in St. Francis Convent, where friends may call from 5:30 to 7:30 p.m. today and a rosary will be said at 5 p.m. Thallemer, Goethals & Wells Funeral Home is handling arrangements.
