SOUTH BEND - Beverly Jane Jerzakowski, 74, died at 7:44 a.m. Thursday in her home. Survivors include her husband, Wally; daughters, Connie (Terry) Griffith and Virginia Jacobs, both of South Bend, Monica Jerzakowski and Tammy Daniels, both of Buena Park, CA, and Karen (Phillip) Hemminger of Ft. Wayne; and two half brothers, Howard and Charles. A Memorial service will be held at 6:30 p.m. Sunday in Seventh Day Adventist Church, 1936 E. Altgeld, South Bend, IN. Elkhart Cremation Services is assisting the family with arrangements.
