CUNNINGHAM, Samuel R. - M.D. - Putnam

Dr. Samuel R. Cunningham, 64, a widely known Oklahoma City surgeon died Monday at St. Anthony Hospital in that city. He was born on a fram in Putnam Co. in 1872. Survivors are two sons; Roger of the home address and Physician (his first name wasn't mentioned), a brother John Cunningham of Indianapolis, sister May Cunningham, also of Indianapolis and his wife.    --- thanks so much to Marilyn W. for her RT obituaries :) Greatly appreciated - kbz
