The Montgomery County Courthouse is located on the corner of Main and Washington. The Micrographics Division has genealogically-oriented records and is located in the basement. Please do not ask too much of this gal (she's WONDERFUL) but you MAY order one/two marriage license, etc. for $1 each. Address is: Montgomery County Clerk, Attention: BJ; 100 E. Main St, Crawfordsville, IN 47933. (They may request as many as they want, it's $1.00 per copy, unless certified then it is $2.00, they also need to know we only accept cash or money orders (NO CHECKS) and a S.A.S.E.)

Where To Get Copies Of Birth
And Death Certificates In


Indiana

Certificates for births or deaths can be obtained from the Local Health Department in the County where the event occurred or by mail, fax or phone from the Indiana State Department of Health. All requests require proper identification. Methods of payment and fees may vary. Please contact the individual County office.

The Indiana State Department of Health can provide a copy of a birth certificate for births in the State of Indiana EXCEPT for births which occurred before October 1907. If birth occurred before this date, contact the health officer in the County where the birth occurred. A $6.00 non-refundable search fee is required before a search can be made, and includes one (1) certified copy if found. There is a $1.00 charge for each additional copy of same record issued at the same time.

Death registration in Indiana began in 1882. Prior to 1900, records of death are filed ONLY with the Local Health Department in the County where the death actually occurred. There are no es to the state death certificates from 1900 through 1918. The specific CITY or County of death must be provided for searches in this period and the search is limited to one County for each Search Fee ($4.00) paid. If the exact date of death is not known, please indicate the five (5) year period to be searched. An additional $4.00 is required for each additional five (5) year period searched, or County SEARCHED from 1900-1918. Search fees are nonrefundable.

OBTAINING A BIRTH CERTIFICATE USING A CREDIT CARD
To obtain a birth certificate using a major credit card (Visa, MasterCard, American Express or Discover), call (317) 233-2700. You will be asked for the following information:

Credit card name, number and expiration date.
Full name at birth
Date of birth.
Place of birth, including (Hospital, City, County and State)
Full name of father
Full name of mother including mother's maiden name.
Purpose for which record is to be used
Relationship to the person whose record is requested
Total Certificates requested and sizes
Daytime phone number and address where to send the birth certificate.
Please ask about turnaround time and possible expediting fee at each location.
