Milton Cushman, being first duly sworn before the Clerk of the Circuit Court of Gibson County aforesaid upon his oath says, that John Wesley Williams departed this life in said County, in testate as he believes on the 5th day of July 1911. Signed, Milton Cushman  Subscribed and sworn to before me the 26th day of June, 1917 signed John T. Ballard, Clerk of the Circuit Court of Gibson County
